Systm Online

Systm Online is available 24 hours a day, 365 days a year and allows you to do the following at a time convenient to you:

* Manage appointments

* View/Request prescriptions

* Views a summary of your patient record

* Add/change your contact details as it is important that these are kept up to date

Patients need to register for this service and this can only be completed at the surgery and not via the telephone as we are required to identify the patient for Data Protection purposes. Photographic evidence such as a passport or driving licence is accepted and once identification is completed the patient will be issued written instructions and a password which they can later amend to their own. Reminders of passwords can be re-issued direct to patients.

The NHS App enables you to:

Serving You

Our dedicated team are here to treat those minor ailments that occur as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or cancel an appointment.

Once registered, patients and healthcare professionals work together to ensure the most appropriate care is provided. This partnership philosophy extends even further and our active patient group exists to make sure that patient needs and the practice offering are always heading in the same direction.

Please note that there may be some conditions that you do not need to see a GP for as a Pharmacist can help you with minor health concerns, and offer clinical advice and over the counter medicines for a range of minor illnesses, such as coughs, colds, sore throats, tummy troubles and aches and pains.

If symptoms suggest it is something more serious, pharmacists have the right training to make sure you will get the help you need. For example, they will tell you if you need to see a GP.

First Contact Practitioner

This First Contact Physiotherapist (FCP) role positions highly skilled and regulated Musculoskeletal (MSK) practitioners at the first point of contact in primary care/GP practices.

This means that a patient presenting with an MSK condition can see an FCP at their first appointment, without seeing the GP first (provided they fit the criteria).  The FCP will provide their MSK Assessment, triage, treatment plan and (where appropriate) investigations and referrals, along with saving both time and resources within primary care/GP practice teams. To do this the FCP will use advanced clinical skills and clinical reasoning.

The FCP will assess a patient to identify any RED FLAGS (serious issues), serious pathology and potential underlying non-MSK related disease, that may present as MSK symptoms and refer appropriately.

Primarily patients should be booked directly into the FCP clinics via the GP reception team.

This service is available for patients aged 16 and over.

Community Pharmacy Consultation Service

What is the Community Pharmacist Consultation Service?

The NHS Community Pharmacist Consultation Service (CPCS) offers patients same day minor illness consultations with a community pharmacist.

This new service can support general practice to ensure patients are seen by the right healthcare professional, in the right place, at the right time and will help make more GP appointments available for those with acute needs.

How does the Service work?

When a patient with minor illness symptoms contacts their GP practice requesting an appointment, the receptionist/care navigator will ask them a series of questions using a standard appropriate symptom checklist. If their symptoms are appropriate, the GP practice can refer them for a same-day consultation with a community pharmacist.  With the patient's consent, the practice team can send an electronic referral message to the pharmacy the patient has chosen, to support safe follow-up and audit.

Following the referral, the pharmacist will contact the patient the same day by telephone and either carry out a consultation by telephone, or arrange for the patient to attend the pharmacy, if appropriate.  Some patients may be offered a video consultation by the pharmacist.

If the pharmacist feels the patient needs to be seen by a GP, they will return the referral back to the GP practice and an appointment will be arranged.

I-Heart Barnsley 365

This service supports GP practice to increase capacity of appointments available to patient in Primary Care. You can also access evening and weekend appointments with GPs and ANPs.

The service is a FREE NHS Service and you do not need to register, and it is available to anyone who is registered with a GP practice in Barnsley.

Named Accountable GP

All patients, on registering with the practice will be allocated a Named Accountable GP who will be responsible for the patient's overall care at the practice. However this does not stop patients from seeing the GP or Nurse etc of their choice at the practice.  However where a patient wants to see a particular GP, they may have to wait longer to see their preferred GP.

Access

The practice premises are Disability Discrimination Act (2005) compliant and are easily accessible for all wheelchairs users.  All patient areas are easily located on the ground floor at all our sites.
