HOW TO ACCESS YOUR RECORDS

Patients have now automatically been given access to their record from February 2023 onwards, as before this was just a summary of any medicals or allergies or medications being taken. This includes letters, test results and appointment notes.

Some people may also have access to information added to their GP record before this date.

You can access your GP record and nominate someone you trust to access it too.  There are 3 main ways to get your GP records:

1. Using your NHS Account

You can get your GP record by logging into your account using the NHS App or NHS website.

Firstly, you need to register for online services and provide who you are.  You can do then when you create an account.

You will need to ask the surgery for online access to your full record, or you still only see your medicines and allergies.

2.  Using other Online Services or Apps

You might be able to use other GP online services and app to get your record.

First, you need to register for online services (SystmOnline) and prove who you are.

You will need to ask the surgery for online access to your fill record, or you will only see medicines and allergies.

3. By Speaking to the Surgery

You can ask for your GP record at your surgery.

CORONAVIRUS VACCINE

All patients wishing to have the Covid Vaccine will now need to either ring 119 or visit the following website: www.nhs.uk.

Please note that the practice does not have any control over the appointments and locations for 119 and are not able to book a Covid Vaccination for you.  We will no longer be contacting patients to book their Covid Vaccinations. 

You may need to demonstrate your Covid-19 Vaccination Status when travelling abroad so please make sure you are prepared and have this before you travel.  Please check the requirements for travel to your chosen destination in advance. 

The NHS Covid Pass lets you share you Covid-19 status records in a secure way.  It allows you to show others the details of your Covid-19 status when travelling abroad to some countries or territories.

You can get an NHS Covid Pass:

* Digitally through the NHS App or online via NHS.UK

* As a letter sent to you in the post if travelling abroad

Please DO NOT contact the surgery about your Covid-19 vaccination status.  GPs cannot provide letters or certificate showing your Covid-19 vaccination status.

NEW EMOTIONAL WELLNESS APP FOR PATIENTS

The new Teladoc myStrength Emotional Wellness App is now available for patients from Consultant Connect.  No referral from a clinician is needed. The App is commissioned by South Yorkshire and Bassetlaw Integrated Care Systems in partnership with Barnsley, Doncaster, Rotherham and Sheffield Clinical Commissioning Groups for people living in these areas.  It is FREE for patient and practices to use.

The app is available to all patients aged 16 years and over to support them in their journey to better emotional health. The customised programme can help with sleep, stress, anxiety and depression.

Sign up to the app requires the user to answer a set of 5 questions to establish whether the service is suitable for them or not.

PATIENT PARTICIPATION GROUP (PPG)

We would like to invite interested patients to join our Patient Participation Group (PPG). 

A Patient Participation Group (PPG) is a group of people who are patients of the surgery and want to help it work as well as it can for patients, doctors and staff. The NHS requires every practice to have a PPG.

Why should I join?

You have been to the surgery as a patient, parent, carer or friend.

Your experience matters and you can bring different ideas to the surgery to help us treat patients better or to improve what we do in some way.

You will also gain a better understanding of the NHS, and gather feedback from other patients.

Will my views be heard?

Your views are important and will be listened to.  It may not be possible to act on ever suggestion, but all feedback is very valuable.

Working in a spirit of mutual respect, openness and trust, all patients' views will be discussed and, where appropriate, we will work together on solutions.
